Athletic Clearance Process: 3 Steps to Complete!
1. Complete the 2013-2014 Athletic Clearance Packet following instructions below:
Click “Submit Form”. Select the appropriate email client. Save completed copy of packet and attach to email addressed to firstname.lastname@example.org
For older versions of Adobe Reader, select “File>Save As”, and save completed copy of form to the computer. Attach completed copy of packet and email to email@example.com
2. Have our Physical Form completed and signed by a Physician. Valid for only one academic school year. A new physical is required each May for the next academic school year.
3. Complete an ImPACT baseline test online:
- Go to: www.impacttestonline.com/schools
- Select "California" and then click on “Launch Baseline Test”
- Enter “0CE92A207D” when asked to enter "Customer ID Code" and then click on "Launch Baseline Test"
