Eligibility
A student-athlete is eligible ONLY after the Pre-participation Physical Evaluation form is signed by a physician and on file with the Athletic Department. All student-athletes' physical
The physical can be done at TKA or by your family physician.
Sports Physical Day Details
As an optional service to families, students can get their physical done at TKA during Sports Physical Day. Space is limited, so please sign up for a time slot to guarantee your spot. (Note: If you completed your physical at TKA's Sports Physical Day in May, you have met the requirement for 2024-2025.)

What to Bring: First page of this Pre-participation Physical Form (History Form) MUST BE COMPLETED AND SIGNED by parent/guardian and turned in on Sports Physical Day.
- What to Wear: All student-athletes should wear long athletic shorts and a t-shirt.
- Allow One Hour: Sports Physical Day is about an hour process to complete. Please allow enough time to complete the physical day process.
