Navigate: School > Web Forms > Family Demographic Form > Student Medical Form
Include physician details, insurance, medications, allergies, and permissions.
- Medications: List each Medication with the dosage and time/frequency. If the student requires an EpiPen, insulin or insulin pump, please indicate that here. Also, be sure to indicate if your student can self-administer medications.
- Medical Conditions: Important to include life-threatening medical conditions and/or allergies.
- Allergies: List all allergies, state the symptoms or reactions to be alerted to, and whether the allergy is life-threatening, requiring an EpiPen as prescribed.
- Over-the-Counter Medications: Click "Yes" to each over-the-counter medication your student is allowed to receive.
SAVE after each entry.

4. Student/Parent Handbook Agreement

TKA offers a number of fun off-campus events each year that help students build relationships with each other and with teachers and staff. Completing this form in the FACTS Family Portal will give your permission for your student to participate in the events scheduled for his/her grade level.

On-Site Counseling Services

TKA is partnering with the Christian Counseling Center of San Jose to offer free, faith-based counseling on campus. More details can be found on the Health and Wellness webpage. Parents who would like their student to take advantage of this service must complete the counseling form on the FACTS Family Portal.

Medication Authorization for Students

If your student has any medication, including over-the-counter medications to be self-administered, a Medication at School Authorization Form is required for each medication. (Exceptions: Tylenol, Advil, Benadryl, Tums, Zyrtec, Claritin, or cough drops may be used if you gave permission for administration in the Portal.)
In addition, if your student has an Epinephrine Auto-Injector or equivalent, a separate Epipen at School Authorization Form is required.

Hard copies of these forms should be dropped off with the TKA nurses in the SAO.

Students may not carry or self-administer medication unless TKA has both:

A physician’s note, and
A parent/guardian’s written authorization

Over-the-counter medications such as Tylenol, Advil, Zyrtec, or cough drops are only permitted if you gave approval in the FACTS Family Portal.

Class Schedules and Textbooks

How to Find Your Student ID Number

Class Schedules

Student class schedules for the 2026–2027 school year will be accessible in the FACTS Family Portal by the end of the day on Friday, August 1. Please log in to review your student's schedule and important placement information.

If your student received a failing grade or did not meet the minimum requirement to advance in a specific subject (such as math or English), their class schedule may have been adjusted over the summer to ensure proper placement.

These adjustments are made thoughtfully to support your student's academic success.

Changing Class Schedules

Schedule changes will only be made if a correction is needed. This is not the time to change your original elective choices or to choose your favorite teacher.

After schedules are released (by the end of the day on Friday, July 31), please use the appropriate form to request a schedule change. Links below will not work until July 31. Schedule corrections should be made prior to picking up textbooks on Monday, August 10. (Note: A very limited number of classes have hard copy textbooks; most classes have digital textbooks.)

Textbook Pick-Up

Most classes at TKA use digital textbooks and provide classroom sets of printed materials. However, a few select courses require physical textbooks.

Picking Up Textbooks:

Students must have any schedule changes completed before picking up books. If you’re waiting on a schedule adjustment, please pick up your textbooks once your schedule is finalized.

- Mon., Aug. 10: 9:00 a.m. -4:00 p.m.
- Tue., Aug. 11: 7:30 a.m. -2:50 p.m. CLOSED during Chapel & after school.

What to Bring:

Before coming to the library, make sure you have a printed paper copy of your schedule. No phone copies, please.
Students, please bring 5 paper grocery bags to the library to help cover textbooks.

Purchasing Textbooks:

Some parents have expressed interest in purchasing textbooks for their students to reduce screen time at home. A document containing the textbook titles and ISBNs for our classes will be available soon.

1. Set Up & Fund Your Account (MySchoolBucks)

Manage lunch balances, set up auto-pay, and view payment history.

Create Your Account: Visit MySchoolBucks.com or download the mobile app.
Link Your Student: Search for the school, then add your student using their name and birthdate, or their School ID.
Payment Methods: Pay online via credit/debit card ($3.75 fee per transaction) or pay by check made out to Epicurean Group (no fee).

2. Pre-Order Weekly Meals (MealOrders)

Select lunches for individual days or the entire week at once.

Create Your Account: Register at MealOrders.com and link your student with their name and classroom/student ID.
Weekly Deadline: Orders must be submitted by Friday at 12:00 PM for the following school week.

Student Photos

The first day of school (Monday, August 10th) features a special morning schedule dedicated to getting everyone’s school photos taken quickly and smoothly.

Student Photo Schedule (Monday, Aug 10)

Students should report for photos according to the schedule below:

7:30 AM – 8:00 AM: Early Bird Session (Open to anyone ready to go!)
8:00 AM – 8:30 AM: 12th Grade (This includes the traditional, fun Senior portrait for the edge of the yearbook!)
8:30 AM – 9:00 AM: 8th & 11th Grade
9:00 AM – 9:30 AM: 9th & 10th Grade
9:30 AM – 9:50 AM: 7th Grade

What You Need to Know

No Pre-Registration Required: Families do not need to sign up online beforehand. Photographers will hand out the necessary paperwork directly to students and staff on-site.
On-Site Support: We will have staff on standby to guide students, keep lines moving, and help the morning run seamlessly.
ID Card Distribution:
- Seniors: Your school IDs are printed in advance and will be ready for immediate pickup.
- All Other Grades: Your IDs will be printed throughout the school day. You can pick them up by grade level during lunch (if ready) or at the end of the day.
Fast & Efficient: We have carefully coordinated our photographer-to-student ratios based on enrollment, ensuring every grade has plenty of time to get their photos taken without rushing.

Lockers

Locker assignments are on the student schedules on the top-left. Students are responsible for bringing their own combination lock. (Note: shackle diameter should not exceed 7 mm and the shackle length should not exceed 19 mm such as Master Lock 1530DCM.) Please keep the combination on file at home. TKA is not liable for property loss or damage. The only tape authorized for use on or in lockers is blue painter's tape, supplied by the student. Please report any locker problems to the SAO.

Parent Volunteer Opportunities

TKA depends on parent support. Whether you’re chaperoning a trip, helping with athletics, or building a set for the school play, your time and talents are needed in order for our school community to thrive. Plus, volunteering is a great way to build relationships with other TKA parents, and with your child's friends.

To be eligible to volunteer or drive for any school activity that involves direct interaction with students, parents must complete our Volunteer Requirements Forms. Completing the forms doesn’t commit you to volunteering for any specific events—you can choose how and when to help.
